When you switch to semitone mode it switches the display on the (Pitchshift) sliders to increment in semitones. If the sliders were originally set to 1.000 then after switching to semitones the sliders will be at something like 0.794 x. Click in the black region of the slider to the right of the slider knob 4 times and the slider knob will increment in semitones up to the value 1.000. By now, the output should sound at the same pitch as the input. To generate the 3rd (e) you need to move the slider up 4 semitones to 1.260 or down 8 semitones to 0.595. Click 4 times in the black area to the right, or 8 times in the black area to the left, will increment the slider a semitone at a time to the value you want.
For the 5th, you do the same but with more semitones.
