V0.9.148, bug fixes & VST debugging.

There's a new version to download. The main thing I've added is some logging to the VST plugin side so hopfully I can narrow down whats causing the crashes a few people are experiencing.

For anyone who is expericing problems could you email to me or post in here the ErrorLog.txt file from the Sonigen modular Data folder, and also a small description of the steps to reproduce it.

The bugs fixed since the last update:

Bug : Menus going off screen. (Hopefully mutiple monitors works now)
Bug : Noise module repeating same pattern each time.
Bug : LFO pitch tracking not scaled correctly.
Bug : Portamento modes fixed, still need to fix ADSFR / mono mode.
Some other minor fixes / tweaks.

I will be focusing on fixing the host crashes and the patches not loading but progress might be a bit slow this week as I will be very busy with my day job. And the fact that I cant reproduce the bugs here makes it more difficult.
